The Smile Man features Sarathkumar as Chidambaram, a retired police officer with Alzheimer's disease. A killer whom the cops name ""Smile Man"" leaves carved smiles on the faces of the people he murders. The main question of the movie is: Who is this killer? In the past, an accident caused Chidambaram's memory problems, and there is an old case involving his missing friend, Venkatesan (Suresh Menon). Venkatesan's son, Arvind, who is also a police officer, believes his father's disappearance is linked to the ""Smile Man"" killer. He also has his suspicions about Chidambaram. Chidambaram tries to follow clues, but he always arrives at the places where the crimes happened after it's too late. It seems like the killer is playing a game with him. Dark pictures, lonely settings, and scary music are part of the movie. Chidambaram's illness is used more as a surprise element than as a way to really explore what it's like to have memory problems.
