The plot of Pelliki Mundu Prema Katha follows Santhosh, also known as “Santhu” (Chethan Cheenu). He is a cheerful young man who falls in love at first sight with a mysterious woman (Sunaina). They spend time together, sharing laughs and deep conversations. They form a connection based on trust rather than appearance. However, just as their feelings grow strong, a major misunderstanding occurs. This makes them part ways with heavy hearts. Months later, thinking he has lost his chance at love, Santhosh agrees to an arranged marriage set up by his family. The story takes a surprising turn. Santhosh and his new bride are both happy to be married. However, what is behind this happiness is a secret. After the wedding, Santhosh begins to notice familiar gestures and habits in his wife that remind him of the woman who hid her face. This familiarity confuses him.
