Asthram is a crime thriller. The story follows inspector Akilan (Shaam), a sharp and dedicated police officer who is currently on medical leave due to a shoulder injury. His quiet recovery is interrupted when a series of disturbing "suicides" occurs across different locations. In each case, the victims are young men with their stomachs slit in a ritualistic, cross-like pattern. The case lacks a clear motive. Akilan is intrigued by that fact. He gets permission from his superior, SP Rajavelu (Aroul D. Shankar), to investigate the case alongside a rookie officer, Sumanth. As Akilan delves deep into the investigation, he finds a chilling common thread: every victim was a skilled chess player. His investigation leads him to an ancient 8th-century book titled Chess. The Secret, which describes a dark Japanese tradition of ritual suicide tied to the game. Akilan identifies a mysterious mastermind operating from the shadows, using a form of high-level mental manipulation or mesmerism to force these individuals into taking their own lives. Akilan is supported by his journalist wife, Priya (Niranjani Ashokan). Together, they handle a web of secrets involving a psychiatrist named Rudhran (Nizhalgal Ravi) and a wealthy businessman, James (Jeeva Ravi). The Who is the elusive killer? How does Akilan find him?
