In a village with clear social divisions, Madhava is a loyal friend and servant to Hema and her father. Hema's father is a former teacher and poet who struggles to publish his classical works. Hema and Madhava have strong feelings for each other but do not realize it because of their differences in caste and class. Hema discovers her love for Madhava during a local drama where she plays Parvati, but she keeps her feelings hidden due to social stigma. Madhava sells his cows to pay for Hema’s sister Lalitha's wedding. In return, Hema’s father offers him his manuscripts to print. However, when Madhava returns from town, he finds out that Hema is being sent to an asylum after a traumatic event that affected her mental health. In the movie " Apathbandhavudu ", Madhava pretends to be mentally unstable to stay close to her and gets admitted to the same asylum. He faces hardships and danger to keep her safe. Just when she starts to regain her memory, a shocking twist occurs. He steps in during an assault and gets accused of wrongdoing himself. Will Madhava be able to save Hema? Can their love overcome the challenges from society? As Hema starts to recover, a key decision awaits. What will they choose, and will their love win against the odds?
