Akaali is a story about Sowmya (Swayam Siddha). She is a newly appointed Investigating Officer sent to the city of Nandesam. She is determined to reopen a mysterious case from seven years ago, which involves a series of suspicious killings. While the local police previously dismissed these deaths, many villagers remain convinced they were the work of an occult group that practices dark rituals. Sowmya actively investigates the cold case files, and she discovers chilling patterns that suggest that the victims were targeted for ritualistic sacrifices. Her investigation leads her to Donald Halloway (Nassar), a retired officer with experience in such cases. Together, they find the existence of a hidden cult known as the Akaali, whose members follow ancient, sinister beliefs. Along the way, they encounter locals like Vijay (Thalaivasal Vijay) and Haszam Rahman (Jayakumar Janakiraman), as well as the mysterious Vincent (Vinoth Kishan), whose past is deeply intertwined with the cult. The investigation becomes a race against time as Sowmya realizes the rituals might still be ongoing. She is forced to battle both human corruption and supernatural forces to stop the cycle of violence.
